Please Complete Back To School Packet ONLINE

Please complete the online Back To School Packet by October 7th! Every year families are asked to complete a Back To School packet and return it to school via their child’s teacher. This year we have made it easier for families by creating a digital version of this packet.

Follow this link to submit your Back To School Packet Today!

The following documents are traditionally included in this packet:

  • K-12 Enrollment Card
  • Universal Form
  • Health Info Exchange Form

We have added the School Counseling Consent Form - used to allow our counselors to speak with your child in the event they have questions.

Parents will complete the entire process online. Simply download and complete the K-12 enrollment form, login to our website, fill out a few questions, and upload the forms.

IMPORTANT: Families who are NEW to Dana Middle and used our online system to enroll their student DO NOT need to complete the Back To School packet. You already completed these steps during the enrollment process!

Pay close attention to what group your student belongs to. The link below will ask you to determine if your student finished their 5th grade school year at Dana Middle last year (GROUP #1 - Returning Students) OR attended and ended their 4th grade school year at one of the elementary schools in the Point Loma Cluster (GROUP #2 - Incoming 5th Grade Students who attended PL Cluster Elementary School)

Homework Club Coming Soon!

We are getting ready to launch Homework Club, our afterschool homework help program. Homework Club is managed by Dana staff members who are available to assist students with homework assignments and projects. Dates and times for Homework Club will be announced shortly along with the sign-up process. Our target launch date is October 11th. Stay tuned!

We would like to thank the Friends of Dana and Correia Middle Schools for making this resource available to Dana students.

EvOLvE Dance Club

Join Dance To EvOLvE at the Dana after school dance club where you get to ‘Dance! Design! Direct!‘.

Classes are every Thursday after school, 12:25-2:25, in the MPR for all students! Do it all in this unique student-directed class that goes beyond traditional dance classes where students are able to make the choices. The EvOLvE class combines dance instruction with creative freedom allowing the dancers to be the decision makers by exploring choreography, music selection, show management, and art and costume design for their end-of-session performance.

Students love this class as it allows peer interaction and truly lets their imagination shine, and parents love this positive and high-energy class because it builds confidence, teamwork and communication skills while also developing dance skills.

So register today, to get dancing, designing, and directing!

SEL Screener Window Oct. 4-8th

This year, our District will implement a social-emotional survey for students as we seek to understand students’ social-emotional needs during this time. The survey will measure students’ competencies in the areas of growth mindset, self-efficacy, self-management and social awareness, including student perceptions on school climate and culture.

This survey will take approximately 25 minutes to complete. Students will take the survey via our District’s Illuminate online platform. Our school and educators will use data from the survey to continue to understand and support students’ needs.

Please see the information that is available to families regarding the survey, survey window and implementation here

All children in grades 4 –12 are scheduled to participate. If you wish for your child to NOT participate in this survey, please contact Vice Principal Perkins at [email protected] by Wednesday, September 29th.

Students Take DEMI Math Assessment on October 5th

We are scheduled to administer the DEMI on Tuesday, October 5th. The results of the DEMI are used to measure Dana Middle against other schools. Math teachers will use the tool to measure student growth throughout the year. Student performance on the DEMI will not impact report card grades. The DEMI is a computer based math assessment to be administered during one period only. The results of the DEMI will be made available to parents by the central office later in the school year.

Our nursing office is in the process of developing a new schedule to accommodate the modified time frame. Students will be brought to the auditorium beginning Period 1. Students will be brought down in groups organized by alphabetical order. Thank you for your patience as we worked with Nursing and Wellness to expand this important program.
